UnitedHealth Foundation’s M Grant Boosts Chronic Disease Care in D.C.

The UnitedHealth Foundation has announced a significant three-year, $3 million grant to Community of Hope (COH), a nonprofit organization based in Washington, D.C., aimed at enhancing and expanding its chronic disease management program. This generous funding will enable COH to further its mission of providing essential healthcare services, housing, and community support throughout the city. Joining us to discuss this initiative are Kathlyn Wee, CEO of UnitedHealthcare Community Plan of Maryland & D.C., and Roxana Trejo, Senior Director of Nursing at Community of Hope.

For nearly 45 years, Community of Hope has been a vital resource in Washington, D.C., serving over 15,000 community members annually through its three community health centers. As a Federally Qualified Health Center, COH ensures that individuals have access to healthcare services regardless of their ability to pay. A key aspect of their work is the chronic disease management program, which offers personalized support through one-on-one engagement with community health workers, nutrition consultations and education, as well as increased clinical engagement and coordination.

The newly awarded grant will significantly enhance COH’s chronic disease management program in several important ways. It will allow the organization to serve more patients, improve health outcomes for those with chronic conditions such as diabetes, hypertension, and asthma, and reduce preventable emergency room visits by fostering better care coordination and patient engagement. Additionally, the grant will help streamline transitions of care for patients who have been hospitalized, ensuring they receive consistent support during recovery.

Chronic conditions are increasingly prevalent, and in D.C., the highest rates are concentrated in Wards 7 and 8. These areas face significant challenges, including the lowest median household income, a shortage of healthcare providers, and a high concentration of Black residents. The funding from the UnitedHealth Foundation will play a crucial role in addressing these disparities and improving the health and wellbeing of those in greatest need.
